Turn “it did not work” into a useful follow-up note

Describe what happened and what was unclear, so the prescribing team can review the plan.

Start with the exact prescription

A useful message begins with the medication name, formulation and directions on the label. Say whether you followed those directions and whether any part was uncertain. If a pharmacy changed the package or the wording looks different from the website, mention that too. The clinician needs to know which product you actually received, not just which brand's advertisement brought you there.

Do not use the follow-up note to design a new dose.
